Yes, referrals for home care services can be received from any medical doctor licensed in the State of New York.

What types of scenarios would qualify a patient for home care services?
Home Care services are generally indicated for patients that are in need of skilled assessment, teaching and treatment. Examples of this include new diabetic patients that require instruction in insulin administration and diabetes management, patients requiring parenteral or enteral therapy, care of a wound, teaching of a medication regime, assessment of compliance with a new medication regime, instruction in gait training following a neurological event or orthopedic surgery.

Do you have special services for HIV/AIDS patients? Does Home Care offer any services for patients who have HIV/AIDS?
HHC Health & Home Care offers a comprehensive range of services for patients with HIV/AIDS. The home care agency is the recipient of Ryan White funding for patients who are either uninsured or underinsured for services they may require. Additionally, home care offers a telehealth program that is specifically designed to improve medication compliance with these patients.

My patient receives outpatient physical therapy in a hospital. Can they receive nursing services simultaneously for the care of a wound on their leg?
Generally when a patient is referred for home care all services such as nursing, physical, occupational and speech therapy are provided by the home health agency. When my patient is receiving home care services how do I find out how they are doing?
Generally, our team of nurses, therapists and social workers will communicate with other members of the health care team on a regular basis to inform them as to the progress of the patient. Frequently members of the health care team contact other members of the health care team to discuss impediments to patients reaching their optimal health status.

A patient requires education in the management of their diabetes and medication. Can Home Care provide this education?
Yes, home care professionals provide ongoing education to patients to assist them and their caregivers in managing their illness with the goal of promoting patient self management.

How do I make a referral to Home Care?
Referrals to home care can be made by calling our toll free number 866-NY-B-HOME, 7 days a week, 24 hrs a day or by sending an email to our referral mailbox @ homecarereferrals@nychhc.org.

How long does it take once a home care referral is made to begin home care services?
Regulations require that a patient receive services within 24 hours of a home care referral unless requested otherwise by the physician or patient.

What specific services does Home Care offer?
Home Care offers nursing, physical, occupational and speech therapy, nutrition services, social work, and home health aide services. Transportation and medical equipment are also provided under arrangement.

Can I get services from HHC Health & Home Care if I only need a home health aide?
No, home health aide service is not a qualifying service for home care. Patients must require the services of a nurse or therapist before home health aide services are provided. When patients no longer require the services of a nurse or therapist the home health aide will be discontinued.

How do patients pay for home care services?
Generally if patients qualify, home care services are reimbursed through Medicare and Medicaid, commercial insurance and many managed care companies. For patients who are uninsured or underinsured provisions can be made to provide patients with the services they require.

How long can a patient stay on Home Care services?
The home care plan is established by the patient’s physician in collaboration with the patient, family and home care agency. The duration of services varies from patient to patient and is based on the patient’s needs and the goals one is looking to achieve.

Do you provide hospice services?
HHC Health and Home Care is not a certified hospice although in certain situations we do provide end of life and palliative care services.

Are social work services provided by Home Care?
Yes, social work services are provided by the home health agency.

Can I call Home Care to order a wheelchair for a patient that is seen in the clinic monthly?
Home Care can work collaboratively with physicians and patients to obtain supplies and equipment. However, the patient must be a patient receiving home care services at the time items are ordered.
