Overview
At HHC Health and Home Care our Behavioral Health Program is designed to help individuals with psychiatric illness manage their care. To accomplish this goal, clients are provided with individually tailored plans of care, which focus on reducing symptoms and helping them function more independently within the community. By coordinating services with the client, family and health care providers, clients are assisted in managing their care within the least restrictive environment-the home.

Admission/Eligibility

For admission to the program the patient must: |
| |
|
| • |
Have a physician-documented psychiatric diagnosis, or exhibit behaviors that warrant the consultation of a psychiatric nurse (i.e., behavior suggesting a mood, thought, or anxiety disorder); |
| • |
Be under the care of a physician; |
| • |
Be in no acute danger to themselves or others; |
| • |
Show potential for improvement/stabilization of symptoms; |
| • |
Reside in Manhattan, Queens, or the Bronx; |
| • |
Be motivated to receive home care services and participate in their plan of care. | |

Skilled Psychiatric Nursing; |

Psychiatric Social Work Services; |
| • |
Physical, Occupational and Speech Therapy; |
| • |
Nutrition Services; |
| • |
Respiratory Therapy; |
| • |
Home Health Aide Services; |
| • |
Durable Medical Equipment. | |

The program is designed to: |

| • |
Reduce hospital length of stay; |
| • |
Reduce the frequency of re-hospitalization; |
| • |
Increase adherence to treatment regimes; |
| • |
Provide education about illness, health maintenance, medications, coping skills, diet, safety issues, use of community resources; |
| • |
Assist individuals with psychiatric illness to reach maximal health, safety and independence within the community setting. | |
